Incident Report Form
(For Team Managers) 

A MESSAGE FROM COBURG BASKETBALL ASSOCIATION
A zero-tolerance policy is applied against any inappropriate or abusive behaviour from carers, spectators, players, coaches and officials to ensure a safe and enjoyable playing experience for all players and participants.  

Complaint Process

At no stage is anyone other than the coach to approach a referee with a complaint.
​

Part of the role of Team Manager is to bring to the attention of the Club any incidents that have not been satisfactory resolved during the game. If you have an issue to report, please speak to the Team Manager of your team.

If there are concerns, the Team Manager is to find the court supervisor (orange top), and discuss with them in a calm manner. If there is no satisfactory resolution, the Team Manager will need to write up an incident report below within 48 hours of the incident for the club to follow up. Please do not contact Coburg Basketball Association directly.

 The report must include the following:  
  • Date of incident:
  • Type of Incident: Referee Issue, Venue Issue, Opposition Issue, Other
  • Time of game:
  • Venue:
  • Teams involved:
  • Names of referees (if known):
